When you’re preparing for dialysis treatment, it may feel like there are lots of details to manage. Your care team will create a master plan to guide you through various aspects of your prescribed treatment and help you stay on track.
This plan is called your Care Plan and includes your dialysis routine, medications, diet, getting active and treatments for other health conditions.

Your doctor and care team will meet with you regularly to review how well your Care Plan is working for you. This is the time to ask questions and share how you’re really feeling – the more details your care team hears from you, the better you’ll be able to tailor your plan.

Speak up about other health conditions
Many people with kidney disease also have other medical conditions, such as diabetes or high blood pressure (hypertension). It’s important to inform your care team about any additional medical conditions or medications. Your care team can then ensure your prescriptions are working together to help you feel your best.
